  • EMPLOYER:
    Copy of identity document.
    Copy of tax code.
    Copy of the Assisted Person's identity document if different from the Employer's.
    Copy of the tax code of the assisted person if referred by the employer
    Medical certification relating to the non-self-sufficient person for whom the request is made
    hiring a carer.
    Landline and mobile telephone numbers and email address for managing the employment relationship.
    Delegation of the employer in case of impossibility to appear at our offices.
  • WORKER:
    Identity card or Passport for foreign citizens. Copy of the tax code.
    If NON-EU citizen, valid RESIDENCE PERMIT or renewal receipt.
    Any diplomas or professional certifications.
    Address abroad or domicile for cohabitants, telephone number and e-mail address.
